Booking and Appointments:
1. By booking an appointment, you agree to these terms and conditions.
2. Appointments can be made via phone, email, or text message.
3. You will receive an appointment reminder 24 hours before your scheduled time.
4. Please confirm your appointment by completing the required pre-appointment documentation or new patient registration forms (if applicable).
Medical Information and Consent:
1. All new clients must complete a comprehensive medical questionnaire prior to treatment.
2. You must inform us of any medications, pregnancy, or ongoing treatments by other healthcare providers.
3. By commencing treatment, you consent to receive the agreed-upon therapy modality.
4. You acknowledge that you have been informed of the potential risks and benefits of the treatment.
Cancellation and No-Show Policy
- Cancellation Notice: If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance.
2. Cancellation Fee: If you cancel or reschedule with less than 24 hours’ notice, you will be charged the full amount for the scheduled appointment.
3. No-Show Policy: Failure to attend a scheduled appointment without prior notification will also result in being charged the full amount for the session.
